We work hard to make what can often be be a very stressful time as smooth as possible. We have a simple six-step process to get started!

01 / CONTACT US

Begin by filling out our contact form or giving us a call. We’ll provide you with an Intake Packet to fill out and other documentation that you will need to provide prior to your intake interview and orientation.

OBTAIN A COURT ORDER

You must begin by obtaining a court order or formal agreement for all parties to take advantage of our services. It must specifically name Unity Care Facility as the service provider for your visitation supervisors.

03 / SCHEDULE INTERVIEW + PAY FEE

Each party involved must submit a non-refundable registration fee of $50.00. A link will be sent to you via Paypal or Square Invoice at least 24 hours before your respective intake interviews.

04 / SUBMIT INTAKE PACKET

Each party involved must fully complete the Intake Packet we provide and submit it to us. This will be due upon your intake interview either at our office or in your home.

05 / INTAKE INTERVIEW

After all documents and information have been submitted to us each party will be contacted individually to set up an intake interview, sign the contract, and go over the visitation process.

06 / SET UP YOUR FIRST VISIT

We’ll set up your first visit!
